Pay by Purchase Order

Only those companies and individuals that currently have an account in good standing with ECS Consulting or Boiler Supply Company, Inc. are authorized to register by way of purchase order. All others wishing to open an account must complete and submit an application for approval prior to registering. To submit a credit application please follow the steps below:

Step 1: Click to (Download Credit Application)

Step 2: Save the file to your desktop

Step 3: Open and fill out the form completely

Step 4: Print out the completed form and sign in the signature field.

Step 5: Send us the completed form in one of 3 ways:

• Scan and email completed and signed form to accounts@boiscotraininggroup.com
• Mail signed and completed form to address at bottom of form, C/O Boisco Training Group
• Fax signed and completed form to 615-248-3794, attention Boisco Training Group
